I believe at one time this did list the funeral home number and the Funeral Home name on the Death Claim submission. This will help the intake team  when they are going to work the quote. it will speed the process up as to having to go and research to see who send the request for a Quote.  If it had that information as well as an email and fax number so we know where to send the request back.
